Responsibilities include for planning, organizing, and directing the home health care and hospice services. A professional nurse uses the resources given to them to plan and direct services that meet the need of the client and their family in the comfort of their homes.

Our licensed practical nurse/vocational nurse is responsible for providing our clients with direct patient care under the supervision of our registered nurses. Patient care, providing treatment, and working with the team are part of their responsibilities in helping the patient reach the outcome they desire.

Responsibilities include the implementation of occupational therapy services and all conditions that are outlined in the service agreement. Certified Occupational Therapy assistants can only perform services that are planned and supervised registered Occupational Therapist.

Responsible for the implementation of standards of care for medical social work services.

They are responsible for planning, organizing, and conducting diet and nutritional education for the client and their family. They function as a member of the home health care and hospice team.

Their primary responsiblity is providing the client’s physical therapy services while adhering to all conditions that are outlined in the personnel services agreement.

The speech language pathologist is responsible for carrying out and implementing the standards of care for speech language pathology services.

The home health aide is a professional member of our home health care and hospice team who, like the LPN, works under the direct supervision of an RN or therapist. They are responsible for performing personal care services that meet the patient’s needs. Home Health Aides are assigned to promote quality and security for the client.

The intake coordinator is responsible for the admission process for new patients by ensuring availability of reimbursement for services, identifying the clinical team to deliver care, and scheduling all patient visits within prescribed discipline and visit frequency requirements.

A receptionist is responsible for welcoming the clients and familiarizing them with the entity’s services, record keeping and management, assisting new agency employees, and handling other administration tasks.
